Checklist for New Patients

  • Your Hint Portal allows you to manage  manage your invoices, payment methods, plan members, and agreements, Health Records will be shared by practice to this Portal as well. Access may be requested HERE

  • If you do not have Health insurance or are looking for a lower cost option, Health Care Cost-Sharing Programs offer a low cost wrap-around solution for larger medical expenses such as hospitalizations or surgeries. These are designed with Direct Primary Care in mind. Consider Zion Health and Sedera.  Ask us to send you a link to enroll if you are interested.

Communication between Patient and Practice

  • As a Member, you have access to the Practice via direct telephone, email, text and video visits on a continuous basis, subject to the hours and limitations outlined below.

    • Non-urgent needs may be communicated by Member via text message to phone number linked to Electronic Medical Record (EMR) (407-499-8012) and will be responded to as soon as possible and within 1 business day.

    • Urgent needs will be communicated by Member via Electronic Medical Record (EMR) text at 407-499-8012 or phone call to same number at Nurse Practitioner at 407-499-8012.  If Patient is experiencing an Urgent matter and it is between the hours of 6 am - 8 pm weekdays or 8 am - 2 pm on weekends, Patient shall text ‘URGENT’ to the EMR number (407-499-8012) text message and then call Joslyn Bear’s Personal Number at 407-252-3276. If Member does not receive a call, text, message back from the practice after 60 minutes, Member agrees to try and call the Practice by phone again. If Provider does not answer or it is outside of these hours and care cannot wait until the next business day, please go to Urgent Care or the Emergency Department.

    • Medical Emergency: If Patient/member is experiencing a Medical Emergency, Patient/member shall call 911 immediately or go to the nearest emergency room.​

Part of the benefit of Direct Primary Care is Continuity of Care when you need it.  We try to discourage hopping on and off the membership. We also understand that life gets in the way sometimes. If you had to cancel and now would like to re-enroll, please pay the $125 re-enrollment fee and sign up below:
