We emphasize personalized and accessible medical services by offering patients a membership-based approach or as we like to call it affordable Concierge Primary Care. This model allows for more direct and frequent interaction between patients and your Nurse Practitioner, resulting in more personalized care.

Joslyn O'Connor Bear, APRN

Joslyn has been a Board Certified Nurse Practitioner in Florida since 2011. She Graduated from Harvard University and University of Central Florida. She has served Pediatric and Adult patients over the last 13 years in both Private Practice as well as in a Community Based Health Center.
Joslyn is passionate about helping patients feel supported and empowering them to be their healthiest. She is committed to preventative care and making sure patients are up to date on all screenings. Joslyn makes Time for YOU so that you can get back to living your life to the fullest with those you love most.
When not in her Nurse Practitioner role, Joslyn can be found spending time outdoors with her husband, 3 children and 2 dogs (one who is a certified Service Dog).

What are the Benefits of Direct Primary Care?
- Affordable monthly membership fee (insurance not accepted).
- Text or Call your Nurse Practitioner as a concern comes up.
- Same or next business day in-office or tele-health visits with no co-pays.
Can I still be a patient even If I have insurance?
- Our patients keep a traditional insurance or health share plan to cover any catastrophic events. And, by switching to a high deductible/lower premium plan, they often save money on healthcare costs—even with their DPC membership included! Switching to a health sharing plan is another great option.
Health Care Cost-Sharing Programs offer a low cost wrap-around solution for larger medical expenses such as hospitalizations or surgeries. These are designed with Direct Primary Care in mind. Consider Zion Health Health Shares.

Do you offer Mental Health Care?
Yes, we offer mental health care with brief counseling sessions, medication management for mental health and collaboration with mental health specialists when needed.
Do you offer Nutrition and Weight Loss Services?
We also offer Nutrition and Wellness Coaching to help patients achieve health and prevent chronic diseases These services include counseling, weight management and lifestyle coaching. By primary care with Nutrition Wellness services, patients can have a more holistic approach to healthcare that addresses their, mental, and emotional-being.
Do you offer Pain Management?
No, chronic pain management will be referred to a specialist.
